Ask a Question

Join PDA Metro Chapter and Dr. James Vesper, PHD for an evening dinner discussion on Human Error — Don’t Blame the Training (because it’s usually something else)

Dr. Vesper will expound on his theory that saying “human error” is the cause of an unwanted event is equivalent to saying; "equipment failure" resulted in a problem. They both are categories that need to be explored in more detail. But often, instead of going deeper to find the true root cause(s), a faster approach is to call into question the knowledge and skills that people have.  In this evening’s presentation we will explore a range of events and see approaches and models that can be used to help investigators find the real cause of an incident and put in place effective corrective actions.
